The best way to communicate your healthcare concerns is via the patient portal. Here you can update your health information, schedule appointments, pay balances owed and send confidential correspondence to Dr. Laura.
The portal is best viewed on a desktop.

If you are unable to complete your paperwork via the patient portal, please complete the following forms prior to your appointment. If you are having difficulty completing both, then please contact the office. If we don't have your forms prior to your appointment, we may have to reschedule.

If you are seeing Dr. Laura in person, make sure to bring these to your appointment. If you have a telemedicine visit, please upload through the patient portal or you can email it to us at admin@springtidefamilyhealth.com.

CANCELLATIONS
We understand that situations come up and cancellations are sometimes unavoidable. However, as a small practice, recurrent last-minute cancellations or no-shows have a large impact. Please give at least 24 hours' notice for cancellations. Three no-shows will result in dismissal from our care.
